Fractures: Bone Repair

4.2K
Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the...

Burn Injuries01:22

Burn Injuries

3.4K
Burn injuries occur when the skin and underlying tissues are damaged due to exposure to heat, electricity, chemicals, radiation, or friction. They can vary in severity, from minor superficial burns to severe deep burns that can be life-threatening.
The damage results in the death of skin cells, which can lead to a massive loss of fluid. Dehydration, electrolyte imbalance, and renal and circulatory failure follow, which can be fatal. Burn patients are treated with intravenous fluids to offset...

Summary

Injured prisoners often have psychiatric issues or engage in violence. Prison shank wounds are less likely to require surgery than similar civilian injuries, indicating potentially less severe trauma in incarcerated individuals.

Area of Science:

  • Trauma Surgery
  • Public Health
  • Criminology

Background:

  • The U.S. prison population has grown, with frequent violent injuries among inmates.
  • Improvised weapons like shanks are common causes of penetrating trauma in prisons.
  • Understanding injury patterns in incarcerated individuals is crucial for trauma care.

Purpose of the Study:

  • To describe injury patterns and characteristics of incarcerated individuals presenting to a trauma center.
  • To compare incarcerated individuals and non-incarcerated individuals with anterior abdominal stab or shank wounds (AASW).

Main Methods:

  • Analysis of injured adult prisoners at a Level 1 trauma center (February 2011–April 2017).
  • Description of prisoner injury characteristics and hospitalizations.
  • Comparison of prisoners with AASW to non-prisoners using statistical tests (chi-square, t-test, logistic/Poisson regression).

Main Results:

  • 2.0% of 14,461 injured adults were incarcerated; 62% involved interpersonal violence, 12% self-inflicted.
  • 33% of injured prisoners had psychiatric disorders.
  • Prisoners with AASW were less likely to undergo laparotomy (42% vs. 67%) or require operative intervention (6% vs. 35%) compared to non-prisoners.

Conclusions:

  • Injured prisoners frequently have psychiatric comorbidities, engage in violence, or self-harm.
  • Prison-related abdominal stab/shank wounds appear less severe, requiring less operative intervention than similar injuries in the general population.
