Summary

Regenerative endodontic procedures (REPs) successfully treated a previously restored mature tooth with apical periodontitis. Long-term follow-up showed functional healing and hard tissue deposition, demonstrating REPs efficacy in retreatment cases.

Background:

  • Regenerative endodontic procedures (REPs) aim to treat apical periodontitis and regenerate the pulp-dentin complex in necrotic teeth.
  • Consensus on REPs for previously treated mature teeth is lacking.
  • This case report details REPs in a previously treated mature tooth with long-term outcomes.

Observation:

  • A 25-year-old woman presented with pain and swelling in a previously endodontically treated mature tooth.
  • The tooth had undergone root canal treatment and restoration 4 years prior.
  • The procedure involved irrigation with sodium hypochlorite and EDTA, calcium hydroxide medicament, induction of apical bleeding, and placement of concentrated growth factors (CGFs).

Findings:

  • The treated tooth remained functional and asymptomatic during 3-year follow-up.
  • Post-first-year, the tooth responded to electric pulp and thermal tests.
  • Radiological assessments confirmed healing of the apical lesion and hard tissue deposition.

Implications:

  • Regenerative endodontic procedures offer advantages over traditional retreatment methods.
  • REPs provide predictable and user-friendly tertiary healing in endodontic retreatment.
  • This case supports the use of REPs for previously treated mature teeth.
