Summary
Frontline health worker (FHW) visits declined but maternal and neonatal health outcomes improved in Bihar, India. Higher quality FHW interactions were linked to better antenatal care, institutional delivery, and early breastfeeding initiation.
Area of Science:
- Public Health
- Health Services Research
- Maternal and Child Health
Background:
- CARE India implemented maternal and neonatal health (MNH) programs in Bihar, evolving from the Integrated Family Health Initiative (IFHI) to a statewide Technical Support Unit (TSU).
- Frontline health worker (FHW) programs are crucial for delivering MNH services through household visits.
Purpose of the Study:
- To assess changes in FHW visit coverage, intensity, and quality between the IFHI and TSU phases.
- To determine the association between FHW outputs and key MNH practices: antenatal care, institutional delivery, and early breastfeeding initiation.
Main Methods:
- Utilized eight rounds of state-representative household surveys (N=73,093 mothers) and two facility assessments (2012-2017).
- Employed logistic regression models to analyze associations between FHW activities and MNH outcomes.
- Compared FHW visit coverage, intensity, and quality across IFHI and TSU program phases.
Main Results:
- FHW visit coverage declined from 60.2% during IFHI to 46.3% during TSU in the intervention districts.
- Despite reduced coverage and quality, MNH outcomes (antenatal care, institutional delivery, early breastfeeding) improved during the TSU phase compared to IFHI.
- Increased FHW visits, particularly in the third trimester, were significantly associated with higher odds of achieving all three MNH outcomes.
- Higher quality FHW interactions, irrespective of visit frequency, were positively associated with improved MNH outcomes.
Conclusions:
- Community-based MNH interventions require continuous strategic evaluation and adaptation for sustained impact.
- The technical support model can be effective in improving MNH outcomes, even with shifts in FHW engagement.
- Ensuring the quality of FHW interactions is as critical as visit frequency for achieving MNH goals.

Community Based Intervention
140
Community-based interventions in mental health represent a paradigm shift from institution-centered care to treatments embedded within the fabric of local communities. By prioritizing inclusion and leveraging existing societal structures, this approach fosters a supportive environment conducive to addressing mental health challenges while promoting individual dignity and agency.

Primary Healthcare Services
1.7K
Primary care promotes wellness and prevents disease. This care includes health promotion, education, protection (such as immunizations), early disease screening, and environmental considerations. Settings providing this type of healthcare include physician offices, public health clinics, school nursing, and community health nursing.

Methods Of Healthcare Delivery System
3.7K
At the different levels of the healthcare system, we see varying methods of healthcare used. These methods include managed care systems, case management, and primary healthcare.
Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is...

Nursing Implementation
5.6K
Implementation is the execution of the nursing care plan developed during the planning phase.
The five steps to implementing effective nursing care include reassessing the patient, reviewing and revising the existing nursing care plan, organizing the resources and care delivery, anticipating and preventing complications, and implementing nursing interventions.

Models of Health Promotion and Illness Prevention II
1.8K
The person's health status fluctuates continually, varying from being in good health to becoming ill and returning to being healthy. To understand the concept of illness prevention, there are two models. First, the health-illness continuum model is a graphic representation of an individual's wellness. It states that a person is considered healthy in the absence of physical disease and the presence of good emotional health.
