Background:

Dedifferentiated liposarcoma (DDLS) typically originates in the retroperitoneum.
Colonic involvement by DDLS is exceptionally rare, with limited documented cases.

Observation:

A case of DDLS presenting as a perforated cecal mass is described.
Rapid tumor development over six months was noted via computed tomography (CT).

Findings:

The patient underwent right hemicolectomy with en bloc resection for the perforated colon mass.
Despite no initial metastatic disease, widespread metastases (liver, lungs, peritoneum) were evident at three-month follow-up.

Implications:

This case underscores the aggressive potential and rapid progression of rare colonic DDLS.
Highlights the importance of early detection and management of unusual gastrointestinal sarcomas.
