Moderate exercise is safe for individuals with Cystathionine β-synthase (CBS) deficiency, as it does not significantly alter plasma total homocysteine (tHcy) levels. This finding suggests physical activity may be a viable option for managing this genetic disorder.

Background:

  • Cystathionine β-synthase (CBS) deficiency causes severe hyperhomocysteinemia and thrombosis.
  • Physical exercise can transiently increase homocysteine in healthy individuals, raising concerns for CBS deficiency patients.

Purpose of the Study:

  • To investigate plasma total homocysteine (tHcy) kinetics during and after moderate-intensity aerobic exercise in patients with homocystinuria.
  • To assess the safety and tolerability of acute exercise in this patient population.

Main Methods:

  • Six adult homocystinuria patients and six controls underwent 30 minutes of moderate aerobic exercise (50W women, 100W men).
  • Blood samples were collected pre-exercise, immediately post-exercise, and at 180 minutes and 24 hours post-exercise.
  • Plasma tHcy, substrate oxidation, and energy expenditure were measured.

Main Results:

  • Exercise was well-tolerated with similar increases in heart rate and energy expenditure in both groups.
  • Patients with homocystinuria had significantly higher baseline tHcy levels than controls (p < 0.05).
  • Exercise did not significantly affect tHcy levels in either patients or controls (p = 0.36).

Conclusions:

  • Acute moderate exercise demonstrates similar metabolic responses and is safe for homocystinuria patients compared to controls.
  • Physical exercise does not significantly alter tHcy concentrations in CBS deficiency patients.
  • Further research is warranted on varying exercise intensities and patient pyridoxine responsiveness.
