Summary

Appendiceal histopathology was found in 25% of endometriosis patients undergoing minimally invasive surgery (MIS). Surgeons should consider concomitant appendectomies during MIS for endometriosis due to high rates and low risks.

Background:

  • Endometriosis is a chronic inflammatory condition affecting women of reproductive age.
  • Appendiceal involvement in endometriosis is uncommon but can present with diverse histopathological findings.
  • Minimally invasive surgery (MIS) is a common approach for endometriosis treatment.

Purpose of the Study:

  • To determine the prevalence of appendiceal histopathology in patients with endometriosis undergoing MIS.
  • To investigate associations between pre-operative symptoms, age, intra-operative findings, and appendiceal histopathology.
  • To evaluate the potential benefit of concomitant appendectomy during MIS for endometriosis.

Main Methods:

  • Retrospective chart review of 135 patients (ages 16-52) with suspected endometriosis undergoing MIS and appendectomy.
  • Data collected included pre-operative symptoms, age, intra-operative appendix appearance, and histopathology results.
  • Analysis focused on patients with histopathologically confirmed endometriosis.

Main Results:

  • 25% of patients with confirmed endometriosis had appendiceal histopathology, including endometriosis (18%), tumors (2%), and inflammation (5%).
  • Dyspareunia (painful intercourse) was the only symptom significantly associated with appendiceal histopathology (p=0.04).
  • Right-sided endometriomas were associated with appendiceal histopathology (p=0.009); age and intra-operative appearance were not significant factors.

Conclusions:

  • A significant proportion of patients with endometriosis exhibit appendiceal histopathology, including tumors.
  • The findings support considering concomitant appendectomy during MIS for endometriosis, given the prevalence and lack of clear predictive factors.
  • This study contributes to understanding appendiceal pathology in endometriosis and informs surgical decision-making.
