This case report details a rare superficial temporal artery aneurysm in a patient with a history of Roux-en-Y bypass and significant weight loss. Surgical excision was successful despite complex aberrant anatomy involving multiple feeding vessels.
Area of Science:
- Vascular Surgery
- Neuroradiology
- Anatomical Studies
Background:
- Superficial temporal artery aneurysms are rare vascular lesions.
- Patient history includes Roux-en-Y gastric bypass and substantial weight loss, potentially influencing vascular integrity.
- Surgical management is indicated for symptomatic or enlarging aneurysms.
Observation:
- A 49-year-old male flight attendant presented with a diagnosed superficial temporal artery aneurysm.
- Imaging confirmed a 1 cm fusiform aneurysm with multiple feeding side branches.
- Intraoperative findings revealed aberrant vascular anatomy complicating dissection.
Findings:
- The superficial temporal artery aneurysm was successfully suture ligated and excised.
- Pathologic analysis confirmed involvement of all three vessel layers, consistent with a true aneurysm.
- The presence of numerous feeding side branches presented a surgical challenge.
Implications:
- This case underscores the importance of detailed preoperative imaging for identifying aberrant vascular anatomy.
- It highlights a rare presentation of a true superficial temporal artery aneurysm.
- Management strategies should consider potential anatomical variations in patients with a history of significant weight loss.
