Tissue Transplantation01:24

Tissue Transplantation

676
Tissue transplantation is a significant medical procedure involving the transfer of cells, tissues, or organs from a donor to a recipient, with the primary aim of restoring lost functions. This procedure is crucial in treating a broad spectrum of diseases, including kidney diseases, liver failure, heart disease, and certain types of cancers.

Kidney Transplant II: Surgical Procedure

128
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Summary

The first Canadian face transplant provided excellent aesthetic outcomes for a patient with severe facial injuries. Despite challenges with immunosuppression and infection, the patient reported high satisfaction and improved function.

Area of Science:

  • Reconstructive surgery
  • Transplantation immunology

Background:

  • Facial vascularized composite allotransplantation (FVCA) offers a novel solution for severe facial disfigurement.
  • This case details the first FVCA performed in Canada.

Observation:

  • A 64-year-old male received a midface allograft following extensive gunshot wound trauma.
  • Surgical planning utilized virtual technology and stereolithographic models.
  • Microsurgical nerve anastomoses were performed to restore sensation and function.

Findings:

  • At 18 months, excellent aesthetic results were achieved with partial sensory restoration.
  • Functional recovery in animation, speech, and mastication showed continuous improvement.
  • The patient was successfully weaned from tracheostomy and gastrostomy tubes.
  • Four episodes of acute rejection were managed effectively.
  • Postoperative infectious complications, including mucormycosis, were treated successfully.

Implications:

  • FVCA is a viable reconstructive option, but immunosuppression remains a significant challenge.
  • Long-term monitoring is crucial for managing immunosuppression-related complications and assessing graft function.
  • Patient satisfaction and community reintegration highlight the procedure's potential benefits.
