Summary

Eating during hemodialysis can increase the risk of symptomatic hypotension in end-stage renal disease patients. Avoiding meals during dialysis is recommended for those susceptible to blood pressure drops.

Background:

  • End-stage renal disease (ESRD) patients undergoing hemodialysis are susceptible to hemodynamic instability.
  • Blood pressure (BP) fluctuations during dialysis can lead to complications such as hypotension.

Purpose of the Study:

  • To investigate the impact of food consumption on blood pressure during hemodialysis sessions.
  • To assess the association between eating and the incidence of symptomatic hypotension in ESRD patients.

Main Methods:

  • A prospective, controlled study involving nine non-diabetic ESRD patients.
  • A standard meal was administered during 62 of 125 hemodialysis treatments.
  • Blood pressure was monitored to compare postprandial and fasting periods.

  • Diastolic and mean BPs decreased significantly faster in the 45-minute postprandial period during fed treatments compared to fasting treatments (P = 0.01 and P = 0.03, respectively).
  • Symptomatic hypotension occurred more frequently in patients who ate during dialysis (13 episodes) versus those who fasted (2 episodes) (P < 0.05).

Conclusions:

  • Consuming meals during hemodialysis is associated with a faster decline in blood pressure and increased risk of symptomatic hypotension.
  • Patients at risk for hypotension during hemodialysis should avoid eating during treatment.
