  • To illustrate the clinical and radiological characteristics of teeth 38 and 48 with acute operculitis.
  • To identify prognostic factors for the progression of acute operculitis.

  • Fifty subjects (19-36 years) diagnosed with acute operculitis were studied.
  • Clinical examinations and orthopantomograms were performed.
  • Four parameters were assessed for prognostic value using the chi-squared test (P<0.05).

Main Results:

  • Vertical angulation, partial (50%) pericoronal flap encapsulation, eruption below the occlusal plane, and impaction by opposing teeth (28 and 18) are key determinants.
  • These factors are associated with the progression of acute pericoronitis.

Conclusions:

  • Prophylactic extraction is recommended for impacted wisdom teeth with acute operculitis to avoid severe complications.
  • Treatment planning should be individualized based on patient presentation.
