Summary

Guided endodontic techniques accurately remove bonded fibre posts, even with dental composite artefacts in cone-beam computed tomography (CBCT) images. This microguided endodontics approach proves predictable and efficient for fibre-post removal.

Background:

  • Removing bonded fibre posts can be challenging due to artefacts in cone-beam computed tomography (CBCT) images caused by composite dental materials.
  • Accurate post removal is crucial for successful endodontic retreatment and preserving tooth structure.

Purpose of the Study:

  • To evaluate the reliability of a guided endodontic technique for removing bonded fibre posts.
  • To assess the accuracy of the technique in the presence of CBCT image artefacts.

Main Methods:

  • Ten simulated models with natural posterior teeth were prepared with forty fibre-post and composite-core restorations.
  • Pre-operative CBCT and optical surface scans were merged for digital design and 3D-printed guides.
  • Two operators used the guides to perform endodontic access, with deviations measured post-operatively.

Main Results:

  • The mean deviation between planned and actual drill paths was 0.39 ± 0.14 mm coronally and 0.40 ± 0.19 mm apically.
