Background:

  • Persistent use of multileaf collimators (MLCs) in VMAT and IMRT leads to mechanical wear, increasing interlocks, breakdowns, and failures.
  • MLC performance and positional accuracy are critical for safe and effective radiation delivery.

Purpose of the Study:

  • To investigate the impact of a comprehensive MLC clean and service procedure on MLC performance and positional accuracy.
  • To assess the effect of this maintenance on unplanned hardware faults and overall linac reliability.

Main Methods:

  • A service and clean procedure was performed on individual MLC leaves of 3 standard and 2 high-definition MLC (HDMLC) Varian TrueBeam linacs.
  • Data analyzed included in-house service logs, Varian MLC backlash tests, daily picket fence QC, and patient delivery trajectory log files.
  • Analysis covered a period of 6 months pre-service and 2 years post-service.

Main Results:

  • Median hardware faults reduced from 12 (pre-clean) to 0-4 (post-clean) at 12 and 24 months for most linacs.

  • MLCs failing daily picket fence QC tolerance (0.25mm) reduced from a median of 14 to 0 post-service; beam hold-offs showed no correlation.

Conclusions:

  • MLC service and cleaning demonstrably improve MLC performance and positional accuracy, reducing reactive maintenance needs.
  • The positive effects were sustained for approximately one year, suggesting a preventative maintenance interval of over two years.
  • This maintenance strategy is crucial for preserving linac function within their expected 10-year lifespan.
