Summary

Deep sclerectomy with suprachoroidal collagen implant (DSSCI) significantly lowers intraocular pressure (IOP) more effectively than intrascleral implantation. Both procedures show similar safety profiles, warranting further investigation into DSSCI efficacy.

Area of Science:

  • Ophthalmology
  • Glaucoma Surgery
  • Biomaterials in Medicine

Background:

  • Deep sclerectomy is a surgical procedure to reduce intraocular pressure (IOP).
  • Collagen implants are used to enhance the efficacy of deep sclerectomy.
  • Comparing intrascleral versus suprachoroidal implantation of collagen requires further study.

Purpose of the Study:

  • To compare the safety and intraocular pressure (IOP)-lowering effects of deep sclerectomy with suprachoroidal collagen implant (DSSCI) versus deep sclerectomy with intrascleral collagen implant (DSCI).

Main Methods:

  • A prospective, randomized, controlled study involving 13 patients with open-angle glaucoma.
  • Each patient received bilateral deep sclerectomy with either an intrascleral or suprachoroidal collagen implant.
  • Long-term follow-up was conducted up to 60 months.

Main Results:

  • Mean postoperative IOP at 60 months was significantly lower with DSSCI (9.8 mmHg) compared to DSCI (14.8 mmHg).
  • Best-corrected visual acuity remained stable post-surgery for both groups.
  • The safety profiles of DSSCI and DSCI were comparable.

Conclusions:

  • Deep sclerectomy with suprachoroidal collagen implant (DSSCI) offers a superior IOP-lowering effect compared to intrascleral implantation.
  • The safety of DSSCI appears similar to DSCI.
  • Larger patient cohorts and extended follow-up are necessary to fully evaluate DSSCI's long-term safety and efficacy.
