Background:

  • Cataract and glaucoma are leading causes of vision impairment, often coexisting.
  • Resource-constrained areas face challenges in managing these conditions effectively.

Purpose of the Study:

  • To describe standardized and simplified modifications to combined trabeculectomy and manual small incision cataract surgery (MSICS).
  • To report on the outcomes of this combined surgical technique in managing coexisting cataracts and glaucoma.

Main Methods:

  • Retrospective analysis of 70 eyes undergoing the modified combined surgery in rural Indian eye care centers (Jan 2018 - May 2019).
  • Success defined as intraocular pressure (IOP) 6-18 mmHg (complete without medication, qualified with ≤3 medications).
  • Surgical procedures were performed by ophthalmology fellows and an experienced glaucoma specialist.

Main Results:

  • At 1-year follow-up, complete success was 55% and qualified success was 83% with minimal medication.
  • Preoperative IOP was 20 mmHg in eyes with advanced glaucoma (82.8%).
  • Success rates did not significantly differ between fellows and the specialist (P>0.44).

Conclusions:

  • A simplified, combined cataract and glaucoma surgery technique is effective.
  • This straightforward approach has potential for wider application, particularly in underserved regions globally.
