Background:

  • Sebaceous neoplasms are uncommon adnexal tumors posing diagnostic challenges.
  • Sebaceous carcinoma co-occurring with sebaceoma is exceptionally rare, with only four prior cases documented.

Observation:

  • A case of sebaceous carcinoma arising from a sebaceoma presented as a solitary posterior neck nodule.
  • Immunohistochemical analysis revealed strong positivity for epithelial membrane antigen and p53 in tumor cells.

Findings:

  • The lesion exhibited distinct benign (sebaceoma) and malignant (sebaceous carcinoma) components, suggesting malignant transformation.
  • The benign component displayed characteristic histopathological features of sebaceoma.

Implications:

  • Adnexal carcinomas may develop through de novo genesis or malignant transformation of benign precursors.
  • Superficial biopsies can be misleading; deep biopsies are essential when clinical suspicion of malignancy exists.
