Tumor Immunotherapy

Immunotherapy is a treatment that boosts or manipulates the immune system to fight diseases, including cancer. For instance, by stimulating an immune response through vaccinations against viruses that cause cancers, like hepatitis B virus and human papillomavirus, these diseases can be prevented. Nonetheless, some cancer cells can avoid the immune system due to their rapid mutation and division. The immune response to many cancers involves three phases: elimination, equilibrium, and escape.

Background:

Thymic neoplasms, particularly thymic epithelial carcinoma, are rare and can exhibit aggressive local invasion and distant metastasis.
A broad range of autoimmune and paraneoplastic syndromes are associated with thymic tumors, sometimes preceding the tumor diagnosis.

Observation:

A 37-year-old woman presented with metastatic malignant thymoma.
She exhibited a unique constellation of autoimmune/paraneoplastic syndromes, including vitiligo, Raynaud's phenomenon, anti-centromere antibodies, eosinophilia, interstitial lung disease, rash, skin thickening, and asymptomatic cryoglobulinemia.

Findings:

The patient's autoimmune manifestations preceded the diagnosis of thymoma, while others were diagnosed concurrently.
Literature review revealed only twenty case reports with three or more concurrent autoimmune/paraneoplastic syndromes, none matching this specific combination.

Implications:

The diverse clinical presentations and features of thymoma can pose diagnostic and therapeutic challenges.
Recognition of unusual paraneoplastic syndromes is crucial for early diagnosis and management of thymic malignancies.
