Summary

Anatomic double-bundle anterior cruciate ligament reconstruction (ACLR) did not show improved outcomes compared to single-bundle ACLR. This study was terminated early due to complications, preventing definitive conclusions on technique efficacy.

Background:

  • Anterior cruciate ligament (ACL) injuries are common in active individuals.
  • Surgical reconstruction aims to restore knee stability and function.
  • Anatomic single-bundle (SB) and double-bundle (DB) techniques are used for ACL reconstruction (ACLR).

Purpose of the Study:

  • To compare clinical outcomes between anatomic SB and DB anterior cruciate ligament reconstruction (ACLR).
  • To test the hypothesis that DB ACLR yields better International Knee Documentation Committee Subjective Knee Form (IKDC-SKF) scores and reduced laxity than SB ACLR.

Main Methods:

  • Active individuals (14-50 years) with recent ACL injury were randomized to SB or DB ACLR.
  • A quadriceps tendon autograft with a patellar bone block was used for both techniques.
  • Primary outcomes included IKDC-SKF scores, KT-1000, and pivot shift tests at 24 months.

Main Results:

  • Study enrollment was suspended due to patellar fractures associated with bone plug harvest.
  • No significant differences in IKDC-SKF, laxity, or return to sport were observed between SB and DB groups at 24 months.
  • Graft rupture occurred in 5.3% of DB and 8.8% of SB recipients; subsequent meniscus injuries occurred in 77.8% of DB and 83.3% of SB patients.

Conclusions:

  • Early study termination due to complications prevented definitive comparison of SB and DB ACLR outcomes.
  • No detectable clinical outcome differences were found between SB and DB ACLR with the specific graft technique used.
  • Further research is needed to evaluate ACLR techniques, considering potential complications.
