Summary

A rare case of papillary carcinoma was found in lateral aberrant ectopic thyroid tissue, distinct from a healthy native thyroid gland. This highlights the importance of considering ectopic thyroid tissue in neck swellings.

Background:

  • Lateral aberrant ectopic thyroid tissue is a rare condition, accounting for 1-3% of all ectopic thyroid.
  • Clinically, these lesions are often misdiagnosed as lymph node swelling or metastatic tumors.
  • Primary carcinoma within lateral aberrant ectopic thyroid tissue, with a normal native thyroid, is exceptionally uncommon.

Observation:

  • A 53-year-old man presented with a large lateral neck swelling.
  • Initial clinical and radiological diagnoses suggested a malignant soft tissue tumor or lymphoma.
  • Fine needle aspiration cytology confirmed metastatic papillary carcinoma.

Findings:

  • Histological examination revealed papillary carcinoma originating in the lateral aberrant ectopic thyroid tissue.
  • The patient's native thyroid gland showed no evidence of malignancy.
  • Surgical management included excision of the mass and subtotal thyroidectomy.

Implications:

  • This case underscores the necessity of including lateral aberrant ectopic thyroid carcinoma in the differential diagnosis of lateral neck masses.
  • Accurate diagnosis and appropriate surgical intervention are crucial for managing this rare condition.
  • Successful treatment involved radioiodine ablation post-surgery.
