Area of Science:

  • Healthcare Quality Improvement
  • Social Determinants of Health Research
  • Patient Navigation

Background:

  • Screening for social determinants of health (SDOH) is crucial for comprehensive patient care.
  • Effective interventions require connecting patients with social service resources.

Purpose of the Study:

  • To develop and implement an agency-specific tool, the Social Screen Assess and Refer (SScAR), for SDOH screening.
  • To integrate the SScAR tool into new patient evaluations.
  • To facilitate referrals to necessary social services for patients identified with needs.

Main Methods:

  • Adapted existing screening tools and created new questions for the SScAR tool, focusing on available social services.
  • Conducted a pilot study to implement the SScAR tool.
  • Measured SDOH screening documentation, tool utilization, number of social needs identified, and referrals made.

Main Results:

  • The SScAR tool significantly improved the documentation of SDOH screening (p < .001).
  • There was a 5% increase in follow-through referrals to specific social resources.

Conclusions:

  • The SScAR tool is a practical method for identifying patient SDOH needs.
  • The tool establishes a system for linking patients to social services.
  • Future research should evaluate the impact of referrals and service utilization on health outcomes.
