Area of Science:

  • Orthopedic Surgery
  • Biomedical Engineering
  • Musculoskeletal Research

Background:

  • Flexor tendon injuries require robust surgical repair to restore function.
  • Traditional tenorrhaphy techniques, such as the modified Kessler (MK) method, have limitations in biomechanical strength and may delay rehabilitation.
  • Novel suture techniques are needed to improve repair integrity and enable earlier patient mobilization.

Purpose of the Study:

  • To develop and evaluate a novel single knot tenorrhaphy (SK) technique for flexor tendon repair.
  • To compare the biomechanical properties of the SK technique against established methods, including the modified Kessler (MK) and double knot Kessler-loop lock (DK) techniques.
  • To assess the ease of performance and time efficiency of the SK technique.

Main Methods:

  • Forty-eight porcine flexor digitorum profundus tendons were repaired using six different techniques: MK, DK, and SK, each with and without epitendinous suture (P).
  • Load-to-failure tests were performed to evaluate biomechanical properties, including ultimate tensile strength and force at 2-mm gap formation.
  • The time required for each repair technique was recorded.

Main Results:

  • The single knot Kessler-loop lock (SK+P) technique demonstrated improved ultimate tensile strength compared to the modified Kessler with epitendinous suture (MK+P).
  • The SK+P method required fewer knots than the double knot Kessler-loop lock with epitendinous suture (DK+P), indicating greater ease of performance.
  • The SK+P repair exhibited increased force at 2-mm gap formation, suggesting enhanced stability.

Conclusions:

  • The novel single knot tenorrhaphy (SK+P) technique offers superior biomechanical properties compared to the traditional modified Kessler method (MK+P).
  • The SK+P technique is easier to perform and provides enhanced repair strength, potentially facilitating earlier mobilization after flexor tendon repair.
  • This improved tenorrhaphy technique holds promise for optimizing patient outcomes in flexor tendon reconstruction.
