Summary

Decompression retinopathy (DR) can cause vein occlusion and choroidal detachment after glaucoma surgery. Prompt management of this rare complication leads to good visual recovery.

A growing pigmented iris lesion in a middle-aged white woman.

Area of Science:

Background:

  • Decompression retinopathy (DR) presents with retinal and vitreous hemorrhage, disc edema, and macular edema.
  • Vein occlusion and choroidal detachment (CD) have not been previously reported as manifestations of DR.

Observation:

  • A case of a 78-year-old male with bilateral primary open-angle glaucoma (POAG) who underwent trabeculectomy with mitomycin C in the left eye.
  • Postoperatively, the patient developed hypotony, followed by CD, vein occlusion, and macular edema six weeks later.
  • The right eye also developed CD after filtration surgery, managed with steroids, leading to resolution in both eyes.

Findings:

  • Glaucoma filtering surgery can lead to decompression retinopathy with venous stasis and choroidal detachment.
