Background:

Purpose of the Study:

  • To report a rare case of non-functional oxyphilic parathyroid carcinoma.
  • To highlight diagnostic challenges and emphasize the need for long-term surveillance.

Main Methods:

  • A 42-year-old male presented with a neck mass and normal thyroid function, calcium, and parathyroid hormone (PTH) levels.
  • Imaging and fine-needle aspiration suggested a follicular thyroid tumor, but left lobectomy revealed parathyroid carcinoma.
  • Immunohistochemistry confirmed tumor cells were positive for PTH and parafibromin, negative for thyroid markers, with a 10% Ki-67 index.

Main Results:

  • Pathological examination revealed capsular and vascular invasion, consistent with malignancy.
  • The diagnosis of non-functional oxyphilic parathyroid carcinoma was established postoperatively.
  • Post-surgery, the patient maintained normal calcium and PTH levels with no signs of recurrence or metastasis after 1.5 years.

Conclusions:

  • Non-functional oxyphilic parathyroid carcinoma is rare, with preoperative diagnosis being challenging.
  • Limited worldwide reports exist, and long-term prognosis data is scarce.
  • Mandatory long-term imaging surveillance is recommended due to the absence of identified recurrence/metastasis markers.
