Physostigmine in mania.

K L Davis, P A Berger, L E Hollister

    Archives of General Psychiatry
    |January 1, 1978

    Summary

Physostigmine salicylate infusion shifted mood and thought content from mania toward depression in most patients with primary affective disorder. However, irritable manics showed minimal changes, suggesting variable responses to cholinergic enhancement.

    Area of Science:

    • Neuroscience
    • Psychiatry
    • Pharmacology

    Background:

    • Primary affective disorder, specifically mania, presents with elevated mood and thought content.
    • Cholinergic system modulation is explored for its potential impact on manic symptoms.

    Purpose of the Study:

    • To investigate the effects of physostigmine salicylate on manic symptoms in patients with primary affective disorder.
    • To evaluate the potential of physostigmine as a treatment to reverse mania.

    Main Methods:

    • A slow intravenous infusion of physostigmine salicylate was administered to eight patients (seven men, one woman) diagnosed with primary affective disorder, mania.
    • Patient mood and thought content were assessed using a visual analog mood scale and the Pettersen scale.

    Main Results:

    • Six out of eight patients experienced a shift from manic symptoms towards depression.
    • Two patients, characterized by irritability, showed minimal changes in hostility, with one experiencing mild depression.

    Conclusions:

    • Physostigmine salicylate can suppress manic symptoms in some patients, indicating a potential role for cholinergic activity in mania.
    • The variable response suggests that the mechanism may involve direct cholinergic effects or interactions with other neurotransmitter systems.
