Depolarizing Blockers: Pharmocokinetics01:19

Depolarizing Blockers: Pharmocokinetics

428
Depolarizing blockers are administered through intravenous injection. Succinylcholine is the most common choice of depolarizing blockers in emergency clinical practices. Although they have a rapid onset, they readily diffuse away from the motor end plate into the extracellular fluid. They are metabolized by enzymes such as liver butyrylcholinesterase and plasma pseudocholinesterases. This produces a short duration of action, typically 5-10 minutes long, unlike nondepolarizing blockers, which...

Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

756
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.
One of the advantages of...

Suctioning the Nasopharyngeal Airway01:29

Suctioning the Nasopharyngeal Airway

1.6K
Nasopharyngeal suctioning is a procedure to remove secretions from the upper part of the respiratory tract that the patient cannot clear independently. It helps maintain airway patency and prevents complications such as aspiration pneumonia.

Summary
This summary is machine-generated.

Pediatric intraocular surgery carries risks similar to adults, including anterior chamber shallowing. A rare case of intraoperative ciliary block in a child was successfully managed with pars plana vitrectomy.

Area of Science:

  • Ophthalmology
  • Pediatric Surgery
  • Glaucoma Management

Background:

  • Intraocular surgery in pediatric patients shares perioperative risks with adults.
  • Assessing anterior chamber shallowing requires a methodical approach in children, mirroring adult protocols.

Observation:

  • A rare case of intraoperative ciliary block occurred during a second glaucoma drainage device implantation in a 3-year-old boy.
  • This complication presented a unique challenge in a pediatric glaucoma patient.

Findings:

  • The intraoperative ciliary block was successfully treated using pars plana vitrectomy.
  • A hyaloid-zonulo-iridectomy was also performed as part of the intervention.

Implications:

  • This case highlights the importance of recognizing and managing ciliary block in pediatric intraocular surgery.
  • Successful treatment with pars plana vitrectomy offers a viable therapeutic option for this rare complication in children.
