Hydatid disease of the spleen is rare but should be considered in endemic areas. Spleen-sparing surgery is recommended to avoid post-splenectomy infections, especially in young patients.

Observation:
- A 28-year-old female presented with an incidentally discovered 7 cm splenic cyst.
- Imaging (ultrasound and CT) confirmed a splenic cyst without other organ involvement.
- Biological confirmation was achieved through indirect hemagglutination.
Findings:
- Spleen-sparing surgery was successfully performed, confirming a hydatid cyst upon examination.
- The patient received a three-month course of albendazole post-operatively.
Implications:
- The rarity of splenic hydatid disease necessitates its inclusion in the differential diagnosis for splenic cystic lesions in endemic areas.
- Spleen-preserving surgical techniques are crucial to prevent post-splenectomy infections, particularly in younger individuals.
