Summary

Posterior scleritis often causes poor vision and is difficult to diagnose. Early detection and treatment improve visual outcomes, but some patients with this eye condition experience vision loss despite therapy.

Background:

  • Posterior scleritis is an inflammatory eye condition affecting the posterior part of the sclera.
  • Diagnosis can be challenging due to varied presentations, often lacking pain or anterior segment involvement.
  • Visual acuity is frequently impaired at presentation.

Purpose of the Study:

  • To review the clinical course, diagnostic challenges, and outcomes of posterior scleritis.
  • To identify common clinical findings, associated systemic disorders, and histopathological features.
  • To evaluate the impact of diagnostic and treatment delays on visual prognosis.

Main Methods:

  • Retrospective review of 47 patients diagnosed with posterior scleritis.
  • Analysis of clinical presentations, fundus findings, diagnostic investigations (including B-scan ultrasound), and histopathology.
  • Correlation of systemic associations, treatment delays, and visual outcomes.

Main Results:

  • 73% of patients presented with visual acuity of 6/18 or less.
  • Common fundus findings included disc swelling, retinal detachment, and macular edema.
  • B-scan ultrasound was the most useful diagnostic investigation; 60% of patients had associated systemic disorders with vasculitis.

Conclusions:

  • Delayed diagnosis and treatment of posterior scleritis negatively impact visual outcomes.
  • Histopathology confirmed scleral vasculitis in affected patients.
  • A subset of patients with no systemic disease may experience irreversible vision loss despite intensive therapy.
