Area of Science:

  • Ophthalmology
  • Surgical Education
  • Medical Training

Background:

  • Phacoemulsification is a critical cataract surgery technique.
  • Assessing surgical learning curves is vital for resident training and patient safety.

Purpose of the Study:

  • To evaluate the phacoemulsification learning curve for ophthalmology residents.
  • To analyze surgical step durations as a function of resident experience.

Main Methods:

  • A cross-sectional study analyzed 528 phacoemulsification surgeries performed by postgraduate year 3 and 4 residents.
  • Surgical step durations (incisions, CCC, hydrodissection, etc.) were recorded and categorized by experience level (A-F).

Main Results:

  • Overall surgical times decreased across experience levels, with significant improvements noted between adjacent categories.
  • Early in training, nucleus disassembly, quadrant removal, and cortical cleanup were the most time-consuming steps.
  • Efficiency gains were sustained through experience category F, indicating a continuous learning process.

Conclusions:

  • A higher surgical caseload, exceeding the 86-case minimum, benefits resident skill development.
  • Significant improvements in operative time for nucleus disassembly and continuous curvilinear capsulorhexis (CCC) were observed after approximately 250 cases.
