Summary

Aortic dissection can cause stroke, but rapid CT imaging protocols can help diagnose it early. Combining head CT, CT perfusion, and neck-to-abdomen CT quickly identifies aortic dissection, enabling timely treatment for stroke patients.

Background:

  • Aortic dissection is a critical cause of stroke, necessitating its exclusion before administering intravenous recombinant tissue plasminogen activator (rt-PA) therapy.
  • Delays in diagnosis due to lengthy imaging can impede crucial surgical treatment for aortic dissection.

Observation:

  • A 65-year-old female presented with stroke symptoms and back pain, initially showing no acute findings on head CT.
  • CT perfusion revealed an ischemic penumbra, and subsequent CT angiography identified Stanford type A aortic dissection extending to the carotid artery.

Findings:

  • Combined CT protocols (head CT, CTP, neck-to-abdomen CT) enabled rapid evaluation for both stroke and aortic dissection.
  • The diagnostic approach identified an aortic dissection as the cause of stroke, allowing for prompt surgical intervention.

Implications:

  • Integrated CT imaging strategies can expedite the diagnosis of aortic dissection in stroke patients.
  • Early detection and intervention for aortic dissection improve patient outcomes and prevent treatment delays.
