Summary

Craniofacial resection (CFR) for tumors with cranial extension carries risks. Intracranial tumor extension and postoperative neurological issues significantly increase perioperative mortality risk in these extensive surgeries.

Background:

  • Craniofacial resection (CFR) is a complex procedure for craniofacial tumors.
  • While oncologically effective, CFRs are associated with significant perioperative risks.

Purpose of the Study:

  • To identify risk factors for perioperative mortality following open craniofacial resection.
  • Focus on deaths occurring during the initial hospitalization.

Main Methods:

  • Retrospective analysis of 102 patients undergoing CFR from May 2009 to December 2018.
  • Data collected from medical records at a tertiary oncology hospital.

Main Results:

  • The majority of patients were male (72.5%), with a mean age of 61.
  • Skin malignancies, primarily squamous cell carcinoma, comprised 63.4% of tumors.
  • Independent risk factors for perioperative death included intracranial tumor extension (HR=4.56) and postoperative neurological dysfunction (HR=10.9).

Conclusions:

  • Tumor extension into the cranium is a significant predictor of perioperative death after CFR.
  • Postoperative neurological complications also independently increase mortality risk.
