Area of Science:

  • Allergy and Immunology
  • Clinical Medicine
  • Toxicology

Background:

  • Fatal food-induced anaphylaxis is a rarely documented clinical event.
  • Anaphylaxis represents a severe, life-threatening allergic reaction.
  • Understanding risk factors is critical for prevention and management.

Observation:

  • Seven cases of fatal food-induced anaphylaxis were identified over 16 months.
  • Victims were atopic, with prior anaphylactic episodes to specific foods (peanut, pecan, crab, fish).
  • Reactions often occurred after food ingestion away from home.

Findings:

  • Contributing factors to fatal outcomes included symptom denial, alcohol consumption, inadequate treatment with oral antihistamines, and glucocorticoid use.
  • Epinephrine was not administered promptly in any of the fatal cases.

Related Experiment Videos

  • Elevated IgE antibodies to the causative food were confirmed in postmortem samples.
  • Implications:

    • Food-allergic individuals must carry and self-administer epinephrine immediately upon symptom onset.
    • Healthcare providers should educate patients on prompt epinephrine use and recognize anaphylaxis severity factors.
    • Emergency medical services should consider cricothyrotomy for airway management in severe anaphylaxis.
