Traumatic internal carotid artery aneurysms are rare but can cause severe bleeding. A microvascular anastomosis can prevent deficits when ligating the carotid artery is necessary.

Area of Science:

  • Neurosurgery
  • Vascular Surgery
  • Trauma Surgery

Background:

  • Traumatic infraclinoidal aneurysms of the internal carotid artery (ICA) are rare vascular lesions.
  • These aneurysms can result from craniocerebral trauma and may lead to delayed, life-threatening hemorrhage into the nasopharyngeal space.

Observation:

  • Patients may present with delayed rupture bleeding months after the initial injury, often requiring acute ENT ward admission.
  • Diagnosis is confirmed via angiography, which is crucial for assessing cerebral collateral circulation.

Findings:

  • In cases of inoperable aneurysms requiring internal carotid artery ligation, extra-intracranial microvascular anastomosis offers improved protection against postoperative neurological deficits.
  • Five patients with infraclinoidal aneurysms were successfully treated with this technique over five years, including two with traumatic aneurysms.

Related Experiment Videos

Implications:

  • This surgical approach provides a safeguard against deficits in complex ICA aneurysm cases.
  • The successful application of extra-intracranial microvascular anastomosis highlights its potential in managing rare traumatic ICA aneurysms.
