Area of Science:

  • Neurology
  • Neurosurgery
  • Otolaryngology

Background:

  • Cholesterol granuloma of the petrous apex is an uncommon condition.
  • Typically, it presents with cranial nerve deficits rather than life-threatening symptoms.

Observation:

  • A case involving a giant cholesterol granuloma causing significant brainstem compression is detailed.
  • This unusual presentation necessitated urgent surgical intervention.

Findings:

  • Surgical decompression resulted in a dramatic improvement of the patient's neurological status.
  • The presented case highlights an atypical, severe manifestation of cholesterol granuloma.

Implications:

  • This case underscores the potential for petrous apex cholesterol granulomas to cause critical mass effect.
  • Prompt surgical management can lead to significant neurological recovery in severe cases.
  • Understanding these rare presentations is crucial for timely diagnosis and treatment.
