  • Elevated serum androgens and a large right adrenal mass (163×110×122 cm) with suspected hepatic invasion were identified.

Findings:

  • The patient underwent surgical resection, with histopathology confirming adrenocortical carcinoma.
  • The tumor was hormone-secreting, leading to the observed virilizing symptoms.

Implications:

  • This case highlights the importance of recognizing virilizing symptoms as a potential indicator of ACC.
  • Prompt diagnosis and surgical intervention are critical for managing ACC and improving patient survival.
  • Multidisciplinary care at specialized centers is essential for optimal follow-up and adjuvant therapy planning.
