Infected simple renal cyst due to Streptococcus pneumoniae rapidly diagnosed by the melting temperature mapping

Abstract

Insights

Infection of a simple renal cyst with Streptococcus pneumoniae is rare in children. A novel Tm mapping method rapidly diagnosed this infection even when cultures were negative, guiding effective treatment.

Background:

Observation:

  • Initial diagnosis suggested pneumonitis or catheter-related infection, but renal cyst enlargement was noted on CT.
  • Blood cultures confirmed invasive pneumococcal infection.

Findings:

  • Despite initial antibiotic treatment, the patient's condition worsened, with CT revealing a complicated renal cyst.
  • Melting temperature (Tm) mapping rapidly detected S. pneumoniae directly from cyst drainage within 4.5 hours.
  • 16S rDNA sequencing confirmed S. pneumoniae in the drainage, despite negative cultures.

Implications:

  • This is the first reported case of a simple renal cyst infected with S. pneumoniae.
  • The Tm mapping method offers a rapid and effective diagnostic approach for challenging infections.
  • Rapid molecular diagnostics can significantly impact antimicrobial stewardship and patient outcomes.
