Summary

Calciphylaxis, or calcific uremic arteriolopathy, is a rare condition causing painful skin necrosis, often in dialysis patients. Early diagnosis and multidisciplinary care can improve quality of life.

Background:

  • Calciphylaxis (calcific uremic arteriolopathy) is a rare, life-threatening vascular disease.
  • Characterized by vascular calcification, occlusion, and painful skin necrosis.
  • Typically affects dialysis patients, with a high mortality rate (up to 80% in 1 year).

Observation:

  • A case of calciphylaxis in a 52-year-old White woman on chronic hemodialysis.
  • Presented with severe, painful subcutaneous nodules and necrotic ulcers on legs.
  • Multiple comorbidities were noted, common in this patient demographic.

Findings:

  • Calciphylaxis pathogenesis is multifactorial and not fully understood.
  • Associated with risk factors like obesity, diabetes, and mineral imbalances (hyperphosphatemia, high PTH).
  • Deficiencies in calcification inhibitors (fetuin-A, matrix Gla protein) may play a role.

Implications:

  • Early diagnosis and interdisciplinary management are crucial.
  • Treatment strategies include pain management, wound care, nutritional support, mineral correction, sodium thiosulfate, and optimized hemodialysis.
  • While prognosis is poor, these interventions can enhance patient quality of life.
