Background:

  • Inferior mesenteric plexus block is indicated for left-sided lower abdominal pain.
  • Terminal cancer patients often present with severe abdominal pain, limiting procedural positioning.
  • Anatomical deformation due to tumor burden complicates standard procedures.

Observation:

  • A 63-year-old male with terminal pancreatic cancer experienced severe left-sided lower abdominal pain (VAS 9/10).
  • The patient was unable to lie prone due to pain and prior hip surgery.
  • Advanced disease with hepatic metastasis, peritoneal seeding, and aortic encasement was noted on CT.

Findings:

  • Initial epidural patient-controlled analgesia allowed partial prone positioning.
  • A CT-guided inferior mesenteric plexus block was successfully performed.
  • Post-procedure, pain was managed with a fentanyl patch (75 mcg/hr), reducing VAS to 4.

Implications:

  • CT guidance enhances safety and accuracy for inferior mesenteric plexus block in challenging cancer patients.
  • This technique facilitates pain management in patients with severe pain and anatomical distortion.
  • Successful pain palliation was achieved, allowing the patient to die without severe pain complaints.
