Summary

Virtual consultations in Traumatology and Orthopaedic Surgery (TOS) show poor diagnostic agreement and high referral rates. Improved coordination and resources are needed between primary and specialized care for effective virtual assessments.

Background:

  • Virtual consultations are increasingly used to bridge primary and specialized care.
  • The effectiveness of virtual consultations in Traumatology and Orthopaedic Surgery (TOS) requires evaluation.

Purpose of the Study:

  • To analyze the current system of virtual consultations in TOS between primary and specialized care.
  • To assess the adequacy of virtual consultations for diagnostic accuracy and treatment planning.

Main Methods:

  • A retrospective observational study of 90 patients with virtual and subsequent face-to-face TOS consultations.
  • Data on diagnostic orientation, history, and tests were evaluated by two independent observers.
  • Comparison of virtual assessment data with final face-to-face evaluations.

  • Low inter-observer agreement on diagnostic orientation, history, and tests during virtual consultations.
  • Insufficient information for decision-making in 52.3% to 41% of virtual requests.
  • High rates of delayed diagnosis (35.2%), new complementary tests (45.5%), and diagnostic discrepancies (30.7%).
  • Significant proportion of patients (51.9%) received no new therapeutic action post-virtual consultation.
  • 34.1% of patients were referred to rehabilitation, indicating potential for optimized referral pathways.

Conclusions:

  • The current virtual consultation model in TOS is inadequate, leading to high rates of unnecessary referrals.
  • Enhanced diagnostic resources for primary care specialists and improved inter-level coordination are crucial.
  • Clearer guidelines are needed to define appropriate conditions and consultations for virtual implementation in TOS.
