Summary

Early non-response to neoadjuvant chemotherapy in cervical cancer patients significantly increases recurrence risk. This finding highlights the importance of monitoring treatment effectiveness early to predict patient outcomes and guide further management strategies.

Background:

  • Neoadjuvant chemotherapy is a standard treatment for cervical cancer.
  • The prognostic significance of early treatment response remains unclear.

Purpose of the Study:

  • To evaluate the impact of early non-response to neoadjuvant chemotherapy on disease-free survival (DFS) in cervical cancer patients.

Main Methods:

  • Systematic literature search of PubMed, Embase, and Cochrane Library up to May 2020.
  • Meta-analysis using random and fixed effects models to pool results.
  • Heterogeneity and publication bias assessed using I2, Cochrane Q, and Egger's/Begg's tests.

Main Results:

  • Analysis included 9 studies (1,462 responders, 490 non-responders).
  • Early non-response was significantly associated with poorer DFS at 1, 3, and 5 years across RECIST and WHO criteria.
  • Hazard ratios for non-response ranged from 0.25 to 0.75, indicating increased recurrence risk.

Conclusions:

  • Early non-response to neoadjuvant chemotherapy is a significant predictor of higher cervical cancer recurrence.
  • Further prospective randomized studies are needed to confirm these findings.
