An inhaled wooden skewer caused a rare pleural empyema after migrating through the airways. This case highlights the diagnostic challenges of deeply lodged foreign bodies in the pleural space.

Background:

  • Foreign bodies in the pleural cavity are uncommon, typically resulting from trauma or medical procedures.
  • Migration of inhaled foreign bodies to the pleural space via bronchopleural fistula is exceptionally rare.

Observation:

  • A case of pleural empyema was caused by an inhaled wooden skewer.
  • Computed tomography (CT) scans and bronchoscopy failed to detect the foreign body due to its peripheral airway migration.

Findings:

  • The wooden skewer perforated the visceral pleura, entering the pleural cavity.
  • The foreign body's thin and pointed nature facilitated its migration and perforation.

Implications:

  • This case underscores the importance of considering unusual foreign body migration in diagnosing pleural infections.
