Cardiovascular disease worsens in patients on long-term hemodialysis, increasing mortality risk. Heart failure with reduced ejection fraction (<50%) is particularly linked to higher mortality rates.

Background:
- Left ventricular (LV) anomalies are common in chronic kidney disease (CKD) and linked to mortality.
- Limited data exists on LV changes during stable, long-term hemodialysis (HD).
Purpose of the Study:
- To investigate the progression of cardiovascular comorbidities and their impact on mortality in stable HD patients over 4 years.
Main Methods:
- A cohort of 1034 stable HD patients from 9 centers was followed for 4 years.
- Yearly echocardiography (2D, M-mode, Doppler) assessed LV structure and function.
- Cardiovascular comorbidities and mortality were recorded.
Main Results:
- Cardiovascular comorbidities significantly increased during follow-up: CAD (73.5% to 88.8%), PAD (29% to 40.9%), CVD (20.4% to 30.8%), VC (65.6% to 89.3%), LVH (67.6% to 76.5%).
- Mortality risk was elevated with CAD, PAD, CVD, and VC.
- Reduced LVEF (<50%) significantly increased mortality risk (1.5 to 2.3-fold).
- A >5% increase in LVEF was associated with higher mortality (1.43-fold adjusted).
Conclusions:
- Cardiovascular disease progresses during long-term HD therapy, elevating mortality risk.
- Heart failure (HF) is highly prevalent, but only HF with LVEF <50% is associated with increased mortality risk.
