Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Pelvic Surgery

Background:

  • Obturator hernias are uncommon abdominal wall defects.
  • This study reviews obturator hernia cases at Mayo Clinic over 15 years.

Observation:

  • Eight patients underwent nine operations for 11 obturator hernias (0.073% of all hernia repairs).
  • Elderly women with chronic conditions were most affected.
  • Mechanical small intestinal obstruction and thigh/groin pain were common symptoms.

Findings:

  • Preoperative diagnosis was rare (1/8 patients); Howship-Romberg sign present in only two.
  • Incarcerated ileum was the most common content; endometriosis found in obturator defects in two patients.
  • Right-sided hernias were more common; bilateral hernias occurred synchronously and metachronously.

Implications:

  • Obturator hernias present diagnostic challenges, often leading to delayed treatment.
  • Patient debilitation and diagnostic delays increase operative morbidity and mortality.
  • Endometriosis is a rare but notable cause of obturator hernia symptoms.
