Summary

Experimental trigeminal pain, specifically in the forehead, increases calcitonin gene-related peptide (CGRP) levels in healthy individuals. This suggests CGRP elevation is a general response to trigeminal nerve stimulation, not exclusive to headache disorders.

Background:

  • Migraine and trigemino-autonomic cephalalgia attacks correlate with elevated alpha-calcitonin gene-related peptide (CGRP) levels.
  • The role of CGRP in trigeminal pain stimulation in individuals without headache disorders remains unclear.

Purpose of the Study:

  • To investigate whether trigeminal pain stimulation in healthy subjects increases CGRP levels.
  • To determine if CGRP elevation is specific to headache disorders or a general response to trigeminal activation.

Main Methods:

  • Eight healthy subjects received subcutaneous injections of capsaicin or sodium chloride in the forehead and mandibular regions.
  • Alpha-calcitonin gene-related peptide (CGRP) levels were measured.
  • Trigeminal-autonomic symptoms like lacrimation and rhinorrhea were monitored.

Main Results:

  • A significant increase in CGRP levels was observed exclusively after capsaicin injection into the forehead (first trigeminal branch).
  • Trigeminal-autonomic activation occurred only with forehead capsaicin injection.
  • No significant changes were noted after sodium chloride injection or capsaicin injection in the mandibular region.

Conclusions:

  • Elevated CGRP levels are not limited to primary headache attacks but also occur after experimental trigeminal pain in the first branch.
  • This suggests CGRP elevation is an unspecific effect of first trigeminal branch stimulation following pain activation.
  • CGRP elevation may not be a specific mechanism unique to idiopathic headache disorders.
