Area of Science:

  • Gastroenterology
  • Vascular Surgery
  • Clinical Medicine

Background:

  • Extensive intestinal ischemia presents a significant diagnostic and therapeutic challenge.
  • Early recognition and prompt management are critical for patient outcomes.

Purpose of the Study:

  • To analyze the clinical features, management, and outcomes of patients with extensive intestinal ischemia.
  • To identify factors influencing survival in this patient cohort.

Main Methods:

  • Retrospective review of 65 patient records diagnosed with extensive intestinal ischemia between 1973 and 1984.
  • Data collected included patient demographics, clinical presentation, diagnostic findings, surgical interventions, and mortality.

Main Results:

  • Pain was the most common symptom. Associated vascular disease, leucocytosis, and metabolic acidosis were frequent findings.
  • The mean interval from admission to operation was significantly shorter in survivors (14.5 h) compared to non-survivors (44 h).
  • Factors associated with increased mortality included older age, delayed laparotomy, and longer lengths of ischemic bowel.

Conclusions:

  • Prompt surgical intervention (laparotomy) is paramount in improving survival rates for extensive intestinal ischemia.
  • Accurate preoperative diagnosis and timely management are essential to reduce the high mortality associated with this condition.
