Area of Science:

  • Gastroenterology
  • Radiology
  • Surgical Pathology

Background:

  • Acute acalculous cholecystitis is difficult to diagnose due to complex clinical scenarios and limited research.
  • Concealed gallbladder perforation is a rare but serious complication.

Observation:

  • An 82-year-old female presented with right upper quadrant pain and local peritonitis.
  • Abdominal ultrasound suggested cholecystitis; CT scan revealed a perforated gallbladder with fluid collection.

Findings:

  • Surgical findings correlated with CT scan results, confirming gallbladder perforation.
  • The patient underwent successful cholecystectomy but later died from pulmonary embolism.

Implications:

  • CT scan is crucial for diagnosing gallbladder perforation, especially with vague abdominal symptoms and sudden resolution.
