Background:

Acute acalculous cholecystitis is difficult to diagnose due to complex clinical scenarios and limited research.
Concealed gallbladder perforation is a rare but serious complication.

Observation:

An 82-year-old female presented with right upper quadrant pain and local peritonitis.
Abdominal ultrasound suggested cholecystitis; CT scan revealed a perforated gallbladder with fluid collection.

Findings:

Surgical findings correlated with CT scan results, confirming gallbladder perforation.
The patient underwent successful cholecystectomy but later died from pulmonary embolism.

Implications:

CT scan is crucial for diagnosing gallbladder perforation, especially with vague abdominal symptoms and sudden resolution.
