Cardiopulmonary Resuscitation II: ACLS Airway Management01:22

Cardiopulmonary Resuscitation II: ACLS Airway Management

226
Airway management is a key skill in emergency and critical care settings, as maintaining a clear airway is essential for adequate oxygenation and ventilation.Head Tilt-Chin Lift TechniqueThe head tilt-chin lift maneuver is an essential technique primarily used in patients without suspected cervical spine injuries. To perform this maneuver, one hand is placed on the patient’s forehead, and gentle pressure is applied backward to tilt the head. The fingertips of the other hand are positioned...

Design Example: Maintaining Level of an Embankment

218
Constructing a roadway embankment over uneven terrain requires precise leveling to ensure stability and proper drainage. Surveyors use a leveling instrument and staff to calculate ground elevations and determine the required fill material at each point along the embankment alignment. The process begins by positioning a leveling instrument near a benchmark with a known elevation. A backsight reading establishes the instrument height, which serves as a reference for subsequent measurements. A...

Upward Impending Motion

424
A square-threaded screw jack is a mechanical device widely used for lifting heavy loads or applying considerable force. Its operation is based on converting the force applied at its handle into a torsional moment, causing the upward impending motion of the screw. This movement is accomplished by overcoming the static friction between the threads of the screw and the jack.
To better comprehend how a screw jack functions, consider the completely unraveled thread as a block in contact with the...

Profile Leveling and Cross Sections01:26

Profile Leveling and Cross Sections

940
Profile leveling and cross-sections are surveying methods used to determine and document terrain elevations for infrastructure projects such as highways, railroads, canals, and pipelines. These methods provide data for earthwork planning and alignment of proposed routes.  Profile leveling involves measuring elevations along a fixed line to create a vertical terrain profile. A surveyor sets up a leveling instrument at the benchmark (BM) and records a backsight (BS) to determine the...

Elevation of Intermediate Points on Vertical Curves01:20

Elevation of Intermediate Points on Vertical Curves

108
Vertical curves are essential in roadway design because they provide smooth transitions between varying roadway grades. Designing vertical curves involves calculating intermediate elevations and identifying the curve's highest or lowest point, which is essential for optimal roadway performance.Intermediate elevations on a vertical curve are determined using the tangent offset method. This method considers the initial elevation at the start of the curve, the grades, and the curve's geometry. The...

Hydraulic Jump: Problem Solving

246
To analyze a hydraulic jump in a rectangular channel with a flow speed of 6 meters per second, follow these steps:Calculate Effective Upstream Velocity:When the downstream gate closes, a hydraulic jump forms, traveling upstream at 2 meters per second. This wave speed combines with the initial channel flow velocity, creating an effective upstream velocity.Identify Flow Velocities Before and After the Hydraulic Jump:Upstream of the hydraulic jump, the effective flow velocity includes both the...

Summary

Helicopter hoist operations (HHOs) can reduce prehospital times in difficult terrain rescues. Implementing HHOs in emergency medical services may improve patient outcomes and access to trauma centers.

Area of Science:

  • Emergency Medicine
  • Aeromedical Services
  • Disaster Response

Background:

  • Helicopter hoist operations (HHOs) are crucial for accessing patients in environments inaccessible by ground.
  • Currently, HHO-equipped helicopters are scarce in German emergency medical services, primarily located in alpine regions.
  • The demand for HHOs in non-alpine difficult terrain remains understudied.

Purpose of the Study:

  • To investigate the demand for HHOs in difficult terrain outside of Germany's alpine regions.
  • To analyze factors influencing prehospital times for rescue operations.
  • To compare the efficiency of HHOs versus ground rescue procedures.

Main Methods:

  • Retrospective analysis of rescue operations in difficult terrain from 2007 to 2018.
  • Evaluation of terrain, operational, demographic, and medical characteristics.
  • Multivariate regression to identify predictors of prolonged response and prehospital times.
  • Simulation study comparing ground versus HHO rescue times using expert surveys and field data.

Main Results:

  • 251 missions were analyzed; most patients were adult males with traumatic injuries.
  • Ground procedures were used in 93% of cases, with significantly shorter prehospital times (72 minutes) compared to actual HHO missions (104 minutes).
  • Simulation indicated HHOs could significantly reduce prehospital time (80 minutes) compared to ground rescue (91 minutes).
  • Prolonged prehospital times were associated with vague accident site descriptions, difficult terrain, distance from roads, slope gradient, and critical medical conditions.

Conclusions:

  • HHOs offer a significant benefit in reducing total prehospital time and facilitating admission to appropriate trauma centers.
  • Standardized dispatch procedures for HHO missions are essential for improving patient outcomes.
  • Expanding HHO capabilities to select emergency medical service helicopters should be considered.
