Background:

Juvenile aponeurotic fibroma (JAF) is a rare fibroblastic tumor predominantly affecting young children.
JAF typically presents on the palms and soles but can occur in various superficial or deep soft tissues.

Observation:

This report details a unique case of JAF occurring within the flexor pollicis longus tendon in an adult patient.
This anatomical location for JAF has not been previously documented in medical literature.

Findings:

Surgical excision of the JAF was performed without compromising the flexor pollicis longus tendon function.
Post-operative magnetic resonance imaging (MRI) at 26 months revealed complete restoration of the tendon's normal contour, indicating successful treatment.

Implications:

This case expands the known anatomical distribution of JAF to include adult tendonous structures.
Successful conservative management highlights the possibility of preserving tendon integrity in rare JAF presentations.
The findings underscore the importance of continued surveillance for recurrence, a known characteristic of JAF.
