Background:

Pouchitis disease activity indices are crucial for clinical trials but lack comprehensive validation.
Existing instruments require assessment of their operating properties and utility as outcome measures.

Purpose of the Study:

To identify and evaluate existing pouchitis disease activity indices.
To assess the operating properties and clinical trial value of these indices.

Main Methods:

Systematic searches of electronic databases for randomized controlled trials and studies on index operating properties.
Analysis of identified indices for clinical, endoscopic, and histologic activity measurement.

Main Results:

Eighteen trials used four composite indices; the Pouchitis Disease Activity Index (PDAI) was most common.
Validity was assessed in 21 studies, with focus on PDAI construct and criterion validity against biomarkers like fecal calprotectin.
Inter-rater reliability and responsiveness of PDAI and modified PDAI (mPDAI) showed fair to moderate results in limited studies.

Conclusions:

Current pouchitis disease activity indices are not fully valid, reliable, or responsive.
There is a clear need for the development and validation of a specific, robust pouchitis disease activity index.
