  • Current fecal biomarkers, like fecal calprotectin, have limitations in sensitivity and specificity for IBD management.
  • There is a critical need for novel, accurate, and non-invasive stool-based biomarkers for IBD.

Purpose of the Study:

  • To identify and validate novel stool protein biomarkers for inflammatory bowel disease (IBD) using a comprehensive aptamer-based screening approach.
  • To evaluate the diagnostic and prognostic performance of identified stool proteins in ulcerative colitis (UC) and Crohn's disease (CD) patients.
  • To compare the efficacy of novel stool biomarkers against fecal calprotectin for IBD disease activity monitoring and prediction of remission.

Main Methods:

  • Aptamer-based screening of 1129 stool proteins from an IBD cohort.
  • Validation of 20 candidate proteins using Enzyme-Linked Immunosorbent Assay (ELISA).
  • Longitudinal analysis of 50 UC patients across four time-points, correlating protein levels with clinical scores (PUCAI, PGA).

Main Results:

  • Ferritin, Fibrinogen, Haptoglobin, Hemoglobin, Lipocalin-2, MMP-12, MMP-9, Myeloperoxidase, PGRP-S, Properdin, Resistin, Serpin A4, and TIMP-1 were significantly elevated in UC and CD patients versus controls.
  • Fecal Fibrinogen, MMP-8, PGRP-S, and TIMP-2 correlated strongly with disease activity scores and outperformed fecal calprotectin.
  • Baseline levels of Fibrinogen, MMP-12, PGRP-S, TIMP-1, and TIMP-2 predicted clinical remission at Week-4, unlike fecal calprotectin.

Conclusions:

  • Aptamer-based screening identified multiple stool proteins with significant potential as IBD biomarkers.
  • Novel stool proteins like Fibrinogen, MMP-12, PGRP-S, TIMP-1, and TIMP-2 demonstrate superior performance over fecal calprotectin for IBD monitoring and predicting remission.
  • These findings support the development of a new generation of non-invasive stool-based diagnostic and prognostic tools for IBD management.
