Background:

  • Hyperglycemic hemichorea (HH) is a rare neurological complication associated with uncontrolled diabetes.
  • Recurrent episodes of HH are exceptionally uncommon, posing diagnostic challenges.

Observation:

  • This study details six cases of HH, with a mean age of 77.2 years (5/6 female).
  • Patients presented with poor glycemic control (HbA1c 9.3%-13%) and basal ganglia hyperintensities on T1-weighted MRI.
  • One patient experienced recurrent hemichorea, ultimately diagnosed with vasculitis after initial resolution with insulin therapy.

Findings:

  • The mean duration of hemichorea was 69 days, with most cases resolving after insulin treatment and some benefiting from dopamine-receptor blockers.
  • Recurrence of hemichorea, even after adequate glycemic control, suggests a potential coexisting condition like vasculitis.

Implications:

  • Hyperglycemic hemichorea generally has a good prognosis with appropriate diabetes management.
  • Clinicians must consider alternative diagnoses, such as vasculitis, in cases of recurrent hemichorea, especially when symptoms persist despite improved glycemic control.
