A 91-year-old man with back pain was diagnosed with Type-B aortic dissection using transthoracic echocardiography (TTE). This imaging technique, utilizing nonconventional windows, proved effective in detecting the aortic dissection.
Area of Science:
- Cardiology
- Vascular Surgery
- Diagnostic Imaging
Background:
- A 91-year-old male with a history of endovascular abdominal aortic repair presented with acute back pain.
- Initial transthoracic echocardiography (TTE) did not reveal abnormalities.
Observation:
- A posterior paraspinal window view during TTE clearly demonstrated an intimal flap in the descending aorta.
- Multiphasic computed tomography (CT) of the aorta confirmed Stanford Type-B aortic dissection.
Findings:
- The patient successfully underwent thoracic endovascular aortic repair (TEVAR) with a descending aorta endoluminal graft.
- The TEVAR procedure created a "full metal jacket" thoracic-abdominal aorta.
Implications:
- This case highlights the diagnostic utility of TTE using nonconventional windows for Type-B aortic dissection.
- Early and accurate diagnosis via TTE can lead to timely intervention and favorable outcomes in aortic emergencies.

TTE is the most common type of echocardiogram which involves placing a transducer on the patient's chest, emitting sound waves to create heart images. TTE is invaluable for evaluating the heart's size, structure, and motion, making it particularly useful for...
